Custom tool files (.opencode/tool/*) are dynamically imported when building the tool registry. A single file that fails to load (e.g. its @opencode-ai/plugin dependency could not be installed) previously died the entire registry and took down every session run in the project. - tool/registry.ts: catch the import failure, record the skipped file, and continue loading the remaining (valid) tool files instead of dying. - session/tools.ts: surface skipped tools as a session.warning event and attach a warning to the assistant message. - schema: add an optional warning field to Assistant messages and a new session.warning event. - app: render tool-load warnings as an inline warning card in the session timeline (same component as errors, warning variant).
