Pasting content doesn't append to existing prompt

If I type a prompt, then ctrl-v to add content, it displaces the existing prompt instead of appending it. So you end up with:
pasted content + earlier prompt

Same root cause as #218: the local echo overlay was holding your typed prompt in the browser while Ctrl+V went through xterm's bracketed paste path straight to the PTY, so the paste physically arrived at codex before the text you had typed. Exactly the "pasted content + earlier prompt" order you saw.

Fixed in v1.12.2 two ways: codex sessions no longer buffer keystrokes at all (typed text reaches codex immediately, so the order cannot invert), and for the CLIs that still use the overlay (claude/gemini/opencode) a bracketed paste now flushes typed-but-unsent text first. Fun detail from testing: codex drops keystrokes that arrive in the same PTY read as a bracketed paste (its paste-burst detection), so the flushed text and the paste are sent as two separate writes 80ms apart.

Update: #240 (just merged) adds predictive write-through echo for Codex on top of the 1.12.2 paste fix. Wire behavior is byte-identical (pinned by an end-to-end trace-equality test), so the paste ordering fix from 1.12.2 is untouched: typed text still goes out before the bracketed paste. The E2E retest for this issue types "abc", pastes "XYZ" and asserts the composer shows exactly "abcXYZ".

@DodgyBadger ships in the next release; a retest on your setup would be great.
