Codex: slash command picker not working

When I type "/" in codex, in a normal terminal you get a preview list which filters as you type or you can scroll through to find the command you want. This does not work in codeman. The slash command works if you know exactly what to type, just not the picker.

Root cause: the local echo overlay buffered your "/" in the browser until Enter, so codex never saw it and never opened the picker. That is also exactly why commands still worked when typed blind: the full "/model" arrived at codex only when you pressed Enter.

Fixed in v1.12.2: codex sessions bypass the overlay, keystrokes reach codex as you type, and the picker pops up and live-filters just like in a plain terminal. Verified against a real codex TUI in a headless browser.

Thanks for this report in particular: "works if you know exactly what to type, just not the picker" was the single most diagnostic sentence across all four issues. Please update and retest.

Update: #240 (just merged) adds predictive write-through echo for Codex on top of the 1.12.2 fix that made the picker work at all ("/" reaching Codex immediately instead of sitting in a buffer). Every keystroke still streams out instantly, so the picker pops and filters live per key; predictions confirm invisibly against the real echo while it does. The E2E retest types "/" then "mo" against a real codex 0.147 TUI and asserts the picker appears and filters.

@DodgyBadger ships in the next release; please give the picker another try then.
