Summary

  • /api/search sends relatedDocuments (top-level field) and smartPanel.relatedDocuments straight to the client from the raw RelatedDocument[] fetched server-side. Each document carries labels: DocumentLabel[], and DocumentLabel includes a server-only owner_id and an arbitrary metadata object — neither was ever redacted before this fix, so every related-document search result exposed internal label ownership UUIDs and raw label metadata to the client.
  • The DB layer's primary RPC path (get_related_document_metadata / _v2) already excludes hidden-status labels server-side, but does not strip owner_id/metadata from the labels it does return — confirmed by reading supabase/schema.sql's function body. This is a currently-live gap on main, not something newly introduced.
  • Adds src/lib/client-source-projection.ts (projectRelatedDocumentForClient, projectDocumentLabelsForClient, projectDocumentLabelForClient) and routes both response fields in src/app/api/search/route.ts through it. Each label is now reduced to { review_status }; hidden labels are dropped as a second line of defense even if the RPC's own filter is ever bypassed (e.g. its fallback query path, which does select(...,owner_id,...,metadata,...) explicitly).
  • Scope is intentionally narrow: this ports only the relatedDocuments projection path out of a much larger, unmergeable WIP branch (PR WIP: Combined Backups for 6 branches #1701) that had accumulated unrelated silent regressions elsewhere. It does not touch smartPanel's other fields, citations, quotes, or best-source recommendations.

Verification

  • npm run test:focused -- --files src/lib/client-source-projection.ts,src/app/api/search/route.ts,tests/client-source-projection.test.ts — new suite (5 tests) plus tests/search-interaction-route.test.ts, tests/search-route-ownership.test.ts, tests/document-enrichment.test.ts, tests/private-access-routes.test.ts (24 + 145 tests) all green; confirmed the new tests actually fail without the fix (asserted against the pre-fix code first)
  • npx eslint src/app/api/search/route.ts src/lib/client-source-projection.ts — clean, 0 warnings
  • npm run format — no changes needed on the touched files (ran before push)
  • Verification not run: npm run typecheck / npm run verify:pr-local — this environment's node_modules is missing tailwind-merge (Node 24.13.0 here vs. jsdom@30's engines requiring >=24.15.0, so npm ci --include=dev is blocked by engine-strict); this blocks typecheck/build for any change in this environment, confirmed by reproducing the identical failure against a clean main checkout with no local edits. CI runs the correct Node version and will validate typecheck/build.
  • UI verification not run: server-only API response shape change, no UI surface touched.

Risk and rollout

  • Risk: Low-medium. Server-only response-shape change to one API route; response now omits fields it should never have sent (owner_id, raw label metadata, hidden labels). No new data is exposed. cover_image_id and table_count are explicitly preserved in the projection to avoid a UI regression (thumbnails / table badges).
  • Rollback: Revert this commit; the route returns to sending raw relatedDocuments/smartPanel.relatedDocuments (restores the leak, not recommended as anything but an emergency rollback).
  • Provider or production effects: None.

Clinical Governance Preflight

  • Source-backed claims still require linked source verification before clinical use — unchanged; citation/chunk fields are untouched.
  • No patient-identifiable document workflow was introduced or expanded without explicit governance approval — this change only removes data from an existing response, it does not add or expand any document workflow.
  • Supabase target remains Clinical KB Database (sjrfecxgysukkwxsowpy) — no Supabase config touched.
  • Service-role keys and private document access remain server-only — this fix strengthens that boundary by ensuring label owner_id (a server-side ownership identifier) no longer reaches the client.
  • Demo/synthetic content remains clearly separated from real clinical sources — not touched by this change.

  • Deployment classification/TGA SaMD impact was checked when clinical decision-support behavior changed — this is a data-exposure fix, not a clinical decision-support behavior change; no SaMD impact.

  • This PR does not touch src/lib/rag/**, retrieval, ranking, or selection — no RAG impact: line required.
  • Follow-up not included here: smartPanel's other sub-objects (bestSource, quotes, citations) may have similar gaps worth auditing separately; scoped out of this PR to keep the fix minimal and reviewable.
