Add Notification Provider Health Checks

Description

Introduce provider-level health information so operators can distinguish a healthy application from an unavailable notification destination.

Acceptance Criteria

Provider health can be checked independently.

External failures are reported without exposing credentials.

Health information is available through an appropriate diagnostic interface.
