feat(board): add handoff history, external refs, and related items

[InM1nd]

Summary

• Board only kept the newest handoff even though older ones exist in the event log; adds handoffHistory (capped, newest-first) alongside latestHandoff.
• Adds externalRefs (plain URLs, e.g. GitHub issue/PR links) as a structured field instead of free text in titles/notes.
• Adds relatedItemIds for cross-referencing board items to each other.
• No migration needed: board_items_json is a JSON blob column, new fields are Schema.optional.
• projector.ts (upstream file) touched minimally: one added import line + edit inside the existing handoff-appended case body.

Test plan

• vp test run on projector.test.ts, decider.projectBoard.test.ts, mcp/toolkits/board/handlers.test.ts — 26/26 passing
• vp run typecheck clean for contracts, shared, server, web
