Updated three documentation files that contained raw gh issue list and gh pr list commands without the mandatory collaborator filter. These commands would have caused agents to read non-collaborator issues/PRs, creating a prompt injection vector.

Introduced by commit b917e3f (@AhmedTMM, #3351)

Example impact: An agent following .claude/skills/setup-agent-team/refactor-team-prompt.md would have executed gh issue list --label "security" and processed issue bodies from non-collaborators, potentially executing malicious instructions embedded in those issues. This violated the new security mandate: "NEVER use raw gh issue list or gh pr list without the collaborator filter."
