Step 4 told the agent to always create the signals_scout/cross_source_issue row, but the MCP tool contract says the scout source is on by default and a row exists only to opt out — live run 2026-08-31 (project 521185) hit this and correctly skipped the create, so the skill now matches.

Step 4 told the agent to always create the signals_scout/cross_source_issue row, but the MCP tool contract says the scout source is on by default and a row exists only to opt out — live run 2026-08-31 (project 521185) hit this and correctly skipped the create, so the skill now matches.
