Hidden error messages when MCP servers fail

• Make sure you are able to repro it on the latest version
• Search the existing issues.

Steps to reproduce

When I use MCP servers, there is a chance an MCP server will fail and the AI just tells me an error occurred but does not give me the information about why.

Expected behavior

The error is shown somehow or included in a log I can see later

Actual behavior

Just says an error occured but no additional details are shared

I just checked that we've included all information from a MCP tool call in the response to AI, including the caught exception if the tool call throws. Maybe we can alter the system prompt to instruct the AI output details errors when tool call fails, and see if that works.
