Updates PLAN.md to reflect the repo’s current state at v0.11.1 and restores the open-issues disposition section as a complete, up-to-date status view.

Adds a second table for everything opened after the July review. Item 3's "=2025.12" is left alone: it narrates the misconception that produced #86/#87, not the current baseline.
