Describe the bug
During a use test, when they searched for an address on the staging site, the map didn't come up.
The map was never visible on the survey page.

To Reproduce
Steps to reproduce the behavior:

  1. Go to survey page
  2. No map
  3. Enter address
  4. Nothing happens

Expected behavior
Map shows up
