Add a skillName option to control the generated skill name. The value is written into the SKILL.md frontmatter before installation, since the skills add CLI has no rename flag. When omitted it falls back to apis-<title>, preserving behavior. Also add a minor changeset for the option, and ignore the worma-docs package in the changeset config (not published).
