Include Anthropic's structured output request params in span metadata so output_config and the legacy output_format schema are visible on traced messages.create calls.

Add a focused regression test for metadata extraction plus a VCR-backed integration test that reproduces the issue with a real structured-output request.

Include Anthropic's structured output request params in span metadata so output_config and the legacy output_format schema are visible on traced messages.create calls.
