Implements issue #42: explicit review prompt selectors, document-only Markdown review, editable default prompt export, and document-specific fix prompt.\n\nChecks: go test ./internal/config; make docs-lint; git diff --check. Other Go test binaries are blocked by macOS dyld missing LC_UUID.
