IX fixes:
ae14019
- refactor initialization/fix initial values
- use global `fEnableInstantX`
- rpc output: `confirmations` (i.e. total) and `bcconfirmations` (blockchain only), fixes#593 also
- throw error in UI before submiting IX if it violates max amount
