test(e2e): Add node-express-streaming E2E test app - #20684

Clone of node-express with span streaming enabled (traceLifecycle: 'stream' + spanStreamingIntegration()). Tests use waitForStreamedSpan/waitForStreamedSpans helpers instead of waitForTransaction. Covers HTTP server spans, error handling, outgoing fetch, request header attributes, tRPC instrumentation, and MCP via StreamableHTTP transport. SSE MCP test is skipped — MCP handler spans are not emitted as streamed spans with SSE transport (only the POST /messages HTTP span arrives).
