CORE-2320: Update Jam SDK docs links and repo metadata - #12

Point npm package links to jam.dev/docs/custom-logs, update the repository URL to github.com/jamdotdev/sdk, drop the live demo link, and rename "Jam.dev SDK" to "Jam SDK".
