Module replacements 3.x is stable now.

📚 Description

This bumps the package to 3.0.0 rather than the beta release.

Oddly, pnpm decided to also update some h3 lockfile stuff. Not sure why, but I suspect its because the lockfile was relying on someone's stale node_modules.
