I am planning to revise the local setup docs to encourage the centralised dev tool that loves in doc-base as a follow up to these changes. But the changes LGTM.

Update docs and comments in various places, and also remove or update recently changed code around DTD entities.
The more notable change is the complere erasing of manual.xml, funcindex.xml and entities/global.ent, both files and file mentions in code.
Plan to merge this in one week. Reviews and comments welcome.
