feat(usage): show live provider limits

[aGamingGod1234]

Provider subscription limits are currently separate from T3 Code's historical token and cost usage, which makes it hard to see remaining Codex and Claude capacity before a provider blocks new work.

This adds a normalized provider-quota contract and cached server service, reads Codex quota and banked resets, tracks Claude quota events, and publishes resilient live snapshots without leaking provider error details. The web client polls the primary environment, shows concise remaining-capacity indicators in the sidebar, and integrates detailed live limits at the top of the existing Usage page.

The follow-up reliability fixes from the fork are included: stable provider identities, safe fallback copies, serialized refreshes, interrupted Codex read recovery, reset handling, and route/live-refresh coverage.

Related: #1732, #6080

Validation:

• 132 focused tests passed
• scoped server, web, and contracts typechecks passed
• all 56 changed files passed formatting and changed-source lint checks

Provider subscription limits are currently separate from T3 Code's historical token and cost usage, which makes it hard to see remaining Codex and Claude capacity before a provider blocks new work.

This adds a normalized provider-quota contract and cached server service, reads Codex quota and banked resets, tracks Claude quota events, and publishes resilient live snapshots without leaking provider error details. The web client polls the primary environment, shows concise remaining-capacity indicators in the sidebar, and integrates detailed live limits at the top of the existing Usage page.

The follow-up reliability fixes from the fork are included: stable provider identities, safe fallback copies, serialized refreshes, interrupted Codex read recovery, reset handling, and route/live-refresh coverage.

Related: #1732, #6080

Validation:

• 132 focused tests passed
• scoped server, web, and contracts typechecks passed
• all 56 changed files passed formatting and changed-source lint checks

Implemented with GPT-5.6 Sol in T3 Code through the Codex harness.
