Refs #233.

Three porch state commits were written after#285 merged, so they were never in it and are stranded on builder/bugfix-260:

The same #233 pattern: porch records the merge, the gate approval and protocol completion into status.yaml at moments that all fall after the PR carrying the work has already been merged, so nothing left is going to carry them.

What this changes

One file, codev/projects/bugfix-260-a-session-refusal-is-raised-an/status.yaml: phase: pr → verified, the pr gate pending → approved with its approval record, the pr_history entry gaining merged: true and its timestamp, and pr_ready_for_human: true → false.

No code, no tests, no docs. origin/main was merged into the branch first, so the diff is that file alone rather than showing unrelated commits as deletions.
