gitHubPullRequestBrowserUrl gates on identity.provider === "github", so the
fallback added in #134 never appears for a self-hosted GitHub Enterprise host
whose domain has no github DNS label.

Those identities carry provider: "unknown", yet the server serves their pull
requests as GitHub after refineUnknownProjectKinds
(apps/server/src/pullRequest/PullRequestService.ts:493-588). So the detail panel
works normally and only the error view comes up empty — exactly when the user
most needs a way out.

Widening it means deciding what provider: "unknown" should imply for URL
construction, which is a real design call rather than a one-line change: the same
identity could be GitLab or Bitbucket self-hosted, and guessing wrong sends
someone to a 404 on the wrong host.
