Summary:
index should always be smaller than weight.size(0). Adding this check in op_embedding.

This is to avoid wild-addr-read error:

Summary:
index should always be smaller than weight.size(0). Adding this check in `op_embedding`.
This is to avoid wild-addr-read error:
