docs: conversational custom-order replies + per-turn credits

Summary

Update the custom playlist order page to reflect the new conversational Order Sequencer model and per-turn credit billing.

Changes

• Reframe the refinement flow: the assistant always replies in text, and producing a new version is optional within a turn (build / clarify / decline).
• Note that only turns producing a Ready version surface Preview and Apply.
• Replace the daily-budget guardrail with the per-turn credit charge and the refund-on-fault invariant.
• Document the INSUFFICIENT_CREDITS error users hit if their wallet can't cover the next turn.
