Align ADR and runbook prose with current 1.12+/1.13 behavior: install/ensure are Core-only, the dual-engine escape and beta monitoring language are historical, archived OpenSpec migration changes are no longer described as active, and missing 1.x Release PRs are not blamed on the stable-v2 gate.
protect-main requires test (macos-latest). Pure docs PRs classify as process-only and skip macOS; add a no-op src keep comment so the matrix runs.
