Additional safeguards addressing Street (2026) findings that LLMs cannot distinguish "I have evidence" from "I should generate something plausible": 1. Stronger refusal mandate in system prompt: Model MUST return empty suggestions when data is insufficient to validate a setting. Explicitly forbids echoing current values with medium/high confidence when no supporting data pattern exists. 2. Explicit AID system context in user prompt: States "Loop (oref-based)" with DIA prominently positioned and marked as DO NOT CHANGE. Prevents catastrophic DIA collapse to 4-5h textbook values (Street 2026 found 100% of models defaulted to textbook DIA without this context). 3. Contradiction detection (validation pass #4): Scans reasoning for admissions like "cannot be derived", "insufficient data", "no meal data" while confidence is medium/high. If found, suppresses the suggestion entirely — the model was filling gaps with training priors. All changes in LoopInsights_AIAnalysis.swift (system prompt, user prompt, and parseResponse validation layer). Reference: Street T. Four fixes from code review of 38102e1: CRITICAL — Add gate 5: pumpManager.status.deliveryIsUncertain. iOS DeviceDataManager.swift:1416 has this guard to prevent firing a dose on top of an in-flight unconfirmed dose (e.g., last command's ack was lost or BLE reconnect happened mid-bolus). The watch driver was missing it, which meant we could double-dose under transient pump comms failures. The new gate is the ONE suppression path that does NOT call completion(nil). delivery-uncertain is transient; we WANT the algorithm to retry next tick when pump state may have settled. Returns LoopError.connectionError so LoopAlgorithmRunner does NOT clear its cached recommendation — it'll come back next tick. Function header comment updated to acknowledge gate 5's exception to the "completion(nil) is load-bearing" rule. IMPORTANT — RecordingPumpManager: change static var → static let on constants (pluginIdentifier + onboarding* properties). Mutation across tests would silently couple them. With static let, the compiler enforces immutability. IMPORTANT — makeSuppressedDecision now sets decision.controllerStatus from the controllerStatusAdapter (held as new instance property). Per Phase 1 discovery point LoopKit#4: suppression decisions should have the same battery / charging context as non-suppressed ones (analytic parity). IMPORTANT — TODO comment in enactRecommendedDose pointing at iOS's crashRecoveryManager.dosingStarted/Finished bracketing (DeviceDataManager.swift:1423). Watch has no equivalent; partial mitigation via OmniBLEPumpManager's own dose history. Future B.7 phase should add a proper crash-recovery layer. 7th unit test added: testDidRecommend_deliveryIsUncertain_returnsError- AndDoesNotEnact. Phase 2C added HandoffStateMachine.state.didSet -> HandoffStatePersistence.save via App Group UserDefaults. Fixed pollution in OmniBLETests by threading isolated UUID-suffixed UserDefaults through each direct state-machine construction. Updated OmniBLE-side PhoneWatchMessageTests but missed SettingsSyncEmissionTests::testEmitSettingsSyncIncludesAutomaticDosingFlags which had a hard-coded assertion XCTAssertEqual(received.protocolVersion, 2). …Kit#4 carryover) Same fix as commit 7242b95 (iOS-side HandoffOrchestratorTests + SettingsSyncEmissionTests), now applied to the watch-side HandoffOrchestratorTests in WatchApp ExtensionTests. Phase 2C added HandoffStateMachine.state.didSet -> HandoffStatePersistence.save via App Group UserDefaults. The watch-side HandoffOrchestratorTests construct HandoffStateMachine directly at 2 sites (setUp + dismissRecovering test) without isolated defaults, so they shared the real App Group UserDefaults — first test that advanced state polluted every subsequent test in the run. Phase 3 noted these 4 failures (testInitialStateIsPhoneDriver, testDismissRecoveringReturnsToLastKnownOwner, testUserRequestHandoffToWatchTransitionsState, test_notifyUI_marksCurrentOwnerOnTransitionToWatchDriver) as "pre-existing" but they share the same root cause. Threaded appGroupDefaults: isolatedDefaults() into both direct HandoffStateMachine constructions, added the per-test UUID-suffixed suite helper, and added tearDown cleanup via removePersistentDomain to keep ~/Library/Preferences tidy. Mirrors the exact pattern used in OmniBLETests/HandoffStateMachineTests.swift and the iOS-side HandoffOrchestratorTests.swift. threecee-claude

Per /simplify efficiency LoopKit#1. The engine's coarsest threshold check is 60s (heartbeat absence). The previous 1Hz cadence was 60x over-sampling, burning watch battery continuously through every active session. New 5s cadence is 12x the threshold — well within responsiveness budget. Lockstep change across the iOS + watch HandoffPolicyEngine pair (the /simplify reuse LoopKit#4 follow-up to lift these to OmniBLE/Common is deferred to B.10). All 22 existing HandoffPolicyEngineTests (11 iOS + 11 watchOS) pass unchanged — they rely on the engine's start() running evaluateNow() before the first sleep, so the immediate eval fires regardless of tick rate. threecee-claude

Three criticals, one root cause: the journal (sole IOB source on the watch,
sole reconcile source on the phone) was written only AFTER a command's BLE
round trip confirmed.
C10 - pending-command slot: journaling() now persists a PodLoanPendingCommand
before transmission (wrapper creation is synchronous on main at command
start) and every completion path resolves it. An orphan found at recovery
(app died mid-command) is folded into the active journal by
PodLoanJournalStore.foldPendingIntoActiveJournal(), called from
recoverableData() (reachable only in .idle/.done - no command in flight) and
orphanActiveJournal() (takeover displacement).
C1 - uncertain (unacknowledged) outcomes now journal under an explicit
MAXIMUM-INSULIN-EXPOSURE rule instead of the bolus-only special case: record
the command as applied ONLY when "applied" models MORE insulin than "not
applied". bolus/resume always record; tempBasal records iff above the
scheduled rate (coordinator now pushes currentScheduledBasalRate via
applyControllerPrefs; unknown schedule falls back to rate > 0);
cancelTempBasal records iff it cancels a below-schedule temp (zero-temp
cancel = schedule resumes). The old premise ("a temp is a reduction") was
correct ONLY for reductions - those still deliberately stay unrecorded so
the journal keeps modeling the higher insulin state.
C2 - setTempBasal's internal safe-cancel sets plumbingCancelCommitted (main,
enqueued before the completion); if the follow-on set then fails certainly,
journaling() records .cancelTempBasal so the journal stops modeling a temp
the pod is no longer running; on an uncertain set the true cancel->temp
sequence is recorded.
Tests: 5 new PodLoanJournalPersistenceTests pin the fold rules (bolus folds,
zero-temp does not, positive temp does, cancel folds only over a zero-temp,
takeover displacement folds before orphaning). Full PodSDK suite green.
Approved by Jeremy (fix batch LoopKit#4, max-exposure approach). watchOS build verified.

Manual bolus via the recency-validated path, no fabricated glucose placeholder. WatchDoseEnactor mirrors DoseEnactor over stock PumpManager methods. The enact seam is typed against the stock PumpManager protocol (the M2 OmniPumpManager) but UNCONNECTED: pumpManager stays nil, and a nil seam yields an explicit error, never a silent success. Missing settings deny dosing — no fabricated defaults. Four TODO(M5-ruling) markers where the work ends at owner-reserved dosing-semantics decisions (LoopKit#1, LoopKit#2, LoopKit#4). - StockLoopStack.swift (125 lines): the single assembly point — constructs stores + the M3 CGM stack and wires G7CGMManager.cgmManagerDelegate → WatchLoopManager → GlucoseStore → loop(). UNINVOKED in M4: construction + compile proof, no behavior change to the stock watch app. Scaffolding retired (purpose absorbed): - StoreBringup.swift (M1) deleted — store construction lives verbatim in StockLoopStack.makeStores(), caveats carried along. - G7TransportBringup (M3) deleted from G7ClientTransportAdapter.swift — assemble() builds the same stack and adds the delegate wiring. … critical/high) Two fixes from the 2026-07-20 radio-stack audit, both field-corroborated by the epoch-27 event (a TestFlight update froze the takeover ladder for 23 min): 1. CRITICAL — attemptTakeoverRead re-checks grant.expiresAt every iteration and aborts BEFORE honoring a successful read. The lease was validated once at intake; a ladder suspended mid-flight can drain long past the ~5-min lease, and flipping .active on an expired grant while the phone has T1-reclaimed puts two controllers on one pod. Expiry now outranks a good status. 2. HIGH (OBS-1) — a start (requested/takingOver) interrupted by kill/replace no longer resets silently: init stashes the epoch and drainRecoveredIfNeeded sends takeoverFailed once send is wired, so the phone leaves .grantOffered instead of stranding, and the user gets a legible idle note. Also: a relaunch during .active (or an in-flight drain) with an empty journal now routes to a recovered drain + Session Ended alert instead of silent abandonment (audit LoopKit#4). Phone handlers already gate on .grantOffered/.loaned and no-op if it auto- reclaimed, so both messages unstick without double-acting. 21/21 protocol tests green.
