- Broaden except clauses in all three ADK callbacks to catch any exception (not just RuntimeError), making the fail-closed claim true when the server is down, times out, or returns 500. - Add explicit ControlSteerError handling in all callbacks so steer controls produce a user-facing response instead of crashing. - Add defensive RuntimeError in _handle_result when is_safe=False but no deny/steer match exists. - Add timeout to _run_sync thread join with a docstring explaining the sync-callback / async-SDK bridge. - Replace brittle string-based 409 detection in both setup_controls.py with httpx.HTTPStatusError status code check.

We may want to abstract setting up these callbacks with AC overrides and expose a clean interface that one can simply pass these overriden ac+callbacks to agent. But, it can be in next iteration.

Add trimmed READMEs and index entries for the two Google ADK examples (callbacks and decorator) introduced in #69.
