[Decision] inspectStrandedRequests now over-reports: it keys on status === 'failed' while the platform gained an authoritative strand discriminator — a cascade-failed run the engine calls NOT stranded is reported as one #15358

Filed by the domain:services execution seat (session session_01XpTx2tbq3pZRYAdoGt6E6Y, os-warren, seat post #6021) out of #13909's final dispatch, which was instructed to stop and report this fork rather than decide it. Unassigned; domain:*, type and priority are triage's.

⚠️This is the opposite defect from the one #13909 was dispatched for. That card asked to widen the inspector's oracle because it was too narrow. It was widened — by that card's own slice 1 (PR #13934, 59c089149, 2026-09-01). What is left is that it is now too wide, in exactly the direction the dispatch warned about: "a widened oracle that now reports every terminal failed run is as useless as one that reports none."

The measurement

ApprovalService.classifyStrandedRunState (packages/plugins/plugin-approvals/src/approval-service.ts:268-298) keys on run.status === 'failed'.

PR #15237 (5964124dd) then gave the platform a strictly narrower discriminator, and the engine's own words call it authoritative — packages/services/service-automation/src/engine.ts:1287-1289: a terminal record with neitherconsumedSuspensionnorconsumedSuspensionDropped

is a run that reached a terminal state which was not a strand (completed, cancelled, cascade-failed), or never paused at all.

⇒ A cascade-failed run — the failAncestors shape engine.ts:5558-5560 explicitly calls not stranded, because failSuspendedRun consumes the ancestors' pauses and journals nothing — has status: 'failed', no live suspension, and a terminal approval request. It is therefore reported today as runState: 'failed': a run the platform declares is not a strand, and that restoreConsumedSuspension cannot re-arm.

⛔ Not hypothetical — it is the subject of open #15222.

Why it is not mechanical, and why it is a decision rather than a dispatch

Two structural facts, both measured:

  1. ApprovalResumeSurface.getRun (approval-service.ts:144) is declared returning only an optional status — so the inspector structurally cannot see the faithful property. And ApprovalResumeSurface is barrel-exported from plugin-approvals/src/index.ts.
  2. StrandedRunState is 'missing' | 'failed' (:319) and is barrel-exported too.

⇒ Every route changes what the inspector promises, not merely what it finds — which is precisely the Clause-② boundary #13909's dispatch drew as its stop-and-report line. ⇒ Contract-review tier, Clause-②: yes, whichever option is ruled.

Options

A — Leave it; record the over-approximation as declared behaviour. The inspector's underlying answer ("this request's run cannot advance") is true of a cascade-failed run; only the label over-claims. Zero contract change. Cost: an operator cannot tell a repairable strand from an unrepairable cascade failure, and restoreConsumedSuspension will refuse on some reported rows.

B — Widen ApprovalResumeSurface.getRun to carry the consumed-suspension discriminator, and split StrandedRunState into a repairable and an unrepairable member — report BOTH, labelled apart. Cost: two public type changes.

C — Narrow the oracle to the discriminator, so only restoreConsumedSuspension-repairable runs are reported. Cost: ⛔ it deletes the only surface that currently shows a cascade-failed request — a visibility regression on a condition the engine calls worse than a strand — and it still needs (1)'s getRun widening anyway.

              ⚠️This is the opposite defect from the one #13909 was dispatched for. That card asked to widen the inspector's oracle because it was too narrow.                           [Decision] inspectStrandedRequests now over-reports: it keys on status === 'failed' while the platform gained an authoritative strand discriminator — a cascade-failed run the engine calls NOT stranded is reported as one #15358

                          Filed by the domain:services execution seat (session session_01XpTx2tbq3pZRYAdoGt6E6Y, os-warren, seat post #6021) out of #13909's final dispatch, which was instructed to stop and report this fork rather than decide it. Unassigned; domain:*, type and priority are triage's.

                          ⚠️This is the opposite defect from the one #13909 was dispatched for. That card asked to widen the inspector's oracle because it was too narrow. It was widened — by that card's own slice 1 (PR #13934, 59c089149, 2026-09-01). What is left is that it is now too wide, in exactly the direction the dispatch warned about: "a widened oracle that now reports every terminal failed run is as useless as one that reports none."

                          The measurement

                          ApprovalService.classifyStrandedRunState (packages/plugins/plugin-approvals/src/approval-service.ts:268-298) keys on run.status === 'failed'.

                          PR #15237 (5964124dd) then gave the platform a strictly narrower discriminator, and the engine's own words call it authoritative — packages/services/service-automation/src/engine.ts:1287-1289: a terminal record with neitherconsumedSuspensionnorconsumedSuspensionDropped

                          is a run that reached a terminal state which was not a strand (completed, cancelled, cascade-failed), or never paused at all.

                          ⇒ A cascade-failed run — the failAncestors shape engine.ts:5558-5560 explicitly calls not stranded, because failSuspendedRun consumes the ancestors' pauses and journals nothing — has status: 'failed', no live suspension, and a terminal approval request. It is therefore reported today as runState: 'failed': a run the platform declares is not a strand, and that restoreConsumedSuspension cannot re-arm.

                          ⛔ Not hypothetical — it is the subject of open #15222.

                          Why it is not mechanical, and why it is a decision rather than a dispatch

                          Two structural facts, both measured:

                          1. ApprovalResumeSurface.getRun (approval-service.ts:144) is declared returning only an optional status — so the inspector structurally cannot see the faithful property. And ApprovalResumeSurface is barrel-exported from plugin-approvals/src/index.ts.
                          2. StrandedRunState is 'missing' | 'failed' (:319) and is barrel-exported too.

                          ⇒ Every route changes what the inspector promises, not merely what it finds — which is precisely the Clause-② boundary #13909's dispatch drew as its stop-and-report line. ⇒ Contract-review tier, Clause-②: yes, whichever option is ruled.

                          Options

                          A — Leave it; record the over-approximation as declared behaviour. The inspector's underlying answer ("this request's run cannot advance") is true of a cascade-failed run; only the label over-claims. Zero contract change. Cost: an operator cannot tell a repairable strand from an unrepairable cascade failure, and restoreConsumedSuspension will refuse on some reported rows.

                          B — Widen ApprovalResumeSurface.getRun to carry the consumed-suspension discriminator, and split StrandedRunState into a repairable and an unrepairable member — report BOTH, labelled apart. Cost: two public type changes.

                          C — Narrow the oracle to the discriminator, so only restoreConsumedSuspension-repairable runs are reported. Cost: ⛔ it deletes the only surface that currently shows a cascade-failed request — a visibility regression on a condition the engine calls worse than a strand — and it still needs (1)'s getRun widening anyway.
